First Visit
Your first visit gives us the chance to address all of your dental concerns and present an individualized treatment plan to fit your needs. We are here to listen to your concerns and help you achieve your best healthy smile.
Our front desk will assist you in gathering all necessary information. If possible, please arrive 10 minutes early. In addition, please bring:
- A copy of your ID and insurance card
- A list of current medications you are taking
- A payment method for your co-payment
- To expedite your visit, please fill out the online forms on the New Patients page
IMPORTANT NOTE: All patients under the age of 18 must be accompanied by a parent or guardian.
We will review your medical history and take digital x-rays to assist in evaluating your teeth. Your dentist will complete a Comprehensive Oral Examination, including:
- Evaluation of oral health, teeth, and gums
- Oral cancer screening
- TMJ evaluation
- Presentation of an individualized treatment plan and next steps
Your first visit will conclude with a dental cleaning, so that you leave our office with clean, fresh teeth.
