What to Bring to Appointment

Several days prior to your appointment, we will send you via text or email New Patient Forms for you to complete. These forms are also available to complete electronically on our New Patients page. Most patients will return this information to us electronically – we like to have this paperwork a least a couple of days before your appointment so we have time to review.  If you are a returning patient, you will be asked to provide us with any updates to your Health History or Insurance at your visit.

In addition, please bring:

• A copy of your ID and insurance card
• A list of current medications you are taking
• A payment method for your co-payment

IMPORTANT NOTE: All patients under the age of 18 must be accompanied by a parent or guardian.
